First Team Meeting Required
Meet your team! You will have an opportunity to ask questions regarding the trip. Your Trip Leads will share vital information about your trip, ministry goals, itinerary, etc.

Final Team Meeting Required
This meeting will focus on team building aspects for our trip. It'll be a good opportunity to get to know your team members better and grow together all while preparing spiritually for the trip. We will also give out Go bags!

Liability Waiver // Medical Release // Emergency Contact Required
These documents will be handed out at the Interest Meeting. The documents MUST be notarized. Most banks have a notary. A notary will be at the 2nd or 3rd meeting. You will receive a notification prior to the meeting.
Power of Attorney // Required for Minors Only

Legal Release Required
This form will be handed out at the Interest Meeting. This release is also listed under the terms and conditions you agree to upon starting your application. Your signature on this form will be required at the first Team meeting. You can keep this copy.
Requirements
Prerequisites
- Valid Passport - Must not expire within 6 months of Launch Day (Day of Departure) **MUST UPLOAD BY 4/17/25**
- Attend all team meetings - Please refer to booklet and email communication for dates, times, and locations.
- Power of Attorney must be signed and notarized for any minors
- Legal Release must be signed by all participants
- Liability Waiver and Medical Release must be signed AND NOTARIZED by all participants
Responsibilities
- The trip will consist of Ministry Teams (Street Ministry), Dental Projects within the established clinic as well as make-shift locations, Feeding Program and Children's Ministry.
Restrictions
- All minors not accompanied by a parent/guardian must get prior approval from trip leads before application acceptance
- Hep B Vaccine required for all working in the medical clinic.
- Any individual under 18 years of age will not be permitted to work in the clinic.
Preparation
- Complete the Online Application and Wait for Approval.
- Apply for a Passport/Renew Passport if Expires 6 Months Before Date of Departure. **NEED TO HAVE BY 04/17/2025
- Print Out Forms Provided (if not received at interest meeting) to be signed and/or notarized.
- Begin Fundraising for your trip.
- Upload Passport to Service Reef Profile **MUST UPLOAD BY FINAL PAYMENT DUE DATE**
